Please print out and use this Order Form to order recordings through the mail. Please include $3.50 per order for shipping and handling, regardless of order size. Be sure to include your name, postal mailing address and phone number. If paying by check, make payable to "Masters of Harmony". If charging your order, indicate the type of card (MasterCard or Visa) and include card number and its expiration date. You must also write "Authorized Signature" and sign your name on all credit card orders.
Foreign customers should mark their checks or money orders with "U.S. Funds" and include an additional $1.00 for shipping and handling. The distribution, sale or advertising of unofficial recordings is not a representation that the contents of such recordings are suitable for contest use.
